• If you are between the ages of 15 and 69 and are planning to become much more physically active than you are now, please check with your doctor before you start.
  • If you have any of the condition listed below:
    • I have been told by my doctor that I have a heart condition AND I should only do physical activity which is recommended by a doctor.
    • I feel pain in my chest when I am doing physical activity.
    • In the past month, I had chest pain when I was not doing physical activity.
    • I ever lose my balance because of dizziness or lose consciousness.
    • I have a bone or joint problem (eg lower back pain, slipped disc etc) that could be made worse by a change in my physical activity.
    • My doctor is currently prescribing drugs for my blood pressure or heart condition.
    • I do know of ANY OTHER REASON why I should not do physical activity.
  • If you are over 69 years of age, and you are not used to being very active, check with your doctor.
  • If you are not feeling well because of a temporary illness such as a cold or a fever – wait until you are feeling better.
  • If you are or may be pregnant – talk to your doctor before you become more active.
  • Please note: If your health changes during your participation in the programme such that you experienced any of the above condition, tell your doctor. Ask whether you should change your exercise plan.
  • Note: This physical activity clearance is valid for a maximum of 12 months from the date it is completed and it shall be deemed invalid upon subsequent changes in the mentioned health/physical conditions stated above.
