If you are between the ages of 15 and 69 and are planning to become much more physically active than you are now, please check with your doctor before you start.
If you have any of the condition listed below:
I have been told by my doctor that I have a heart condition AND I should only do physical activity which is recommended by a doctor.
I feel pain in my chest when I am doing physical activity.
In the past month, I had chest pain when I was not doing physical activity.
I ever lose my balance because of dizziness or lose consciousness.
I have a bone or joint problem (eg lower back pain, slipped disc etc) that could be made worse by a change in my physical activity.
My doctor is currently prescribing drugs for my blood pressure or heart condition.
I do know of ANY OTHER REASON why I should not do physical activity.
If you are over 69 years of age, and you are not used to being very active, check with your doctor.
If you are not feeling well because of a temporary illness such as a cold or a fever – wait until you are feeling better.
If you are or may be pregnant – talk to your doctor before you become more active.
Please note: If your health changes during your participation in the programme such that you experienced any of the above condition, tell your doctor. Ask whether you should change your exercise plan.
Note: This physical activity clearance is valid for a maximum of 12 months from the date it is completed and it shall be deemed invalid upon subsequent changes in the mentioned health/physical conditions stated above.
